Family Care Teams

The “Family Care Teams Webinar Series” is a set of one-hour webinars designed to support family care team members as they journey towards effective teamwork and seamless collaborative care.

This series is perfect for health care professionals, including family physicians, nurse practitioners, registered nurses, social workers, pharmacists, other allied health professionals and Family Care Team staff who are eager to strengthen their collaborative care skills.

These webinar sessions will be recorded for the purpose of future knowledge sharing. The recordings will be available to other FCT members who may not have been able to attend the live session.

Chronic Disease Management in Family Care Teams: Diabetes Care and Navigation

This is the latest in a series of webinars focusing on chronic disease management in Family Care Teams.  Previous topics in this recurring area include digital screening and assessment tools for chronic disease, and diabetes treatment pathways.

The goal of this webinar is to build awareness of diabetes management services available in Newfoundland & Labrador and enhance capacity for Family Care Teams to manage diabetes cases.

Objectives:       By the end of the webinar, participants will be able to:

  1. Adopt the Diabetes Canada clinical practice guidelines for diabetes care in their Family Care Team practice.
  2. Utilize the web of resources for diabetes clients in Newfoundland and Labrador, specifically diabetes education centers, remote patient monitoring, insulin pump program, and continuous glucose monitoring program.
  3. Discuss and define the roles of diabetes educator and/or chronic disease nurse, and their integration in Family Care Teams’ interprofessional practice.

Integrating Generative Artificial Intelligence in Collaborative Family Care Team Practice

Learn about the capabilities of Gen AI tools available to Family Care Team practitioners and explore their impact on daily collaborative practices in Newfoundland and Labrador’s Family Care Teams.

Goal: To support the implementation of Generative Artificial Intelligence (Gen AI) applications in Family Care Team 

Objectives:

By the end of the webinar, participants will be able to:

  1. Define Generative Artificial Intelligence (Gen AI) and discuss its applications for primary health and social care settings.
  2. Discuss risks to patient safety and privacy when integrating Gen AI tools into primary care service delivery.
  3. Identify and use Gen AI features in CorCare to improve workflow in Family Care Team practice.
  4. Adapt to Gen AI-supported workflows in an interprofessional collaborative primary care practice environment.

 

The format of the webinar will be a panel discussion. We’d like to invite attendees to submit questions for the panel ahead of time.

Gender-Affirming Care in Family Care Teams

Learn about enhancing capacity for gender-affirming care in Newfoundland and Labrador’s Family Care Teams.

Goal: To support participants in providing gender-affirming care for adults aged 18+ through role-specific strategies, fostering collaborative practice, and applying local resources and standards effectively.

Objectives:

By the end of the webinar, participants will be able to:

  • Discuss key concepts in gender diversity and their clinical relevance to gender-affirming care (GAC), including gender identity, gender expression, and sex assigned at birth.
  • Recognize systemic barriers and common clinical harms experienced by gender-diverse patients and describe how GAC improves health outcomes.
  • Summarize the current GAC landscape in Newfoundland and Labrador, including prevalence, referral pathways, MCP coverage, and available local resources.
  • Plan for patient consultations addressing gender-affirming care needs, including initiating respectful conversations, understanding/assessing patient goals (e.g., hormone therapy, surgical readiness, mental health), and applying relevant standards of care.
  • Identify role-specific, team-based approaches for delivering and coordinating GAC, including how to support inclusive environments, documentation, referrals, and continuity of care.
